BREAST RECONSTRUCTION Sub-pectoral silicone gel implants. This approach involves implanting an inflatable bag which is filled with saline via a subcutaneous valve implanted in the patient's side. About 50ml of saline is injected at 1-2 week intervals, until the breast mound is about 1.5 times larger than the remaining breast (so that the breast will hang naturally when the bag is removed). The bag is then removed and replaced with a silicone implant at a second operation. Recent devices, such as the Becker prosthesis, incorporate a silicone prosthesis and so only the valve needs to be removed; this can be done under local anaesthetic.
