Wide Local Excision

The aim of wide local excision is to remove the palpable lesion with a 2 cm margin of normal breast tissue. Approximately 70-75% of patients are suitable for wide local excision and hence breast conservation.

It is however mandatory to discuss advantages as well as disadvantages of breast conservation and following points which are listed below should be carefully explained to the patient:

1. Patients with breast conservation will require radiotherapy to the breast.

2. Multi-focal tumours and those of greater than 4 cm in measurements are generally unsuitable for primary wide local excision.

3. Cosmetic end result depends on technique as well as site of tumour (i.e. central tumours have poor cosmoses) and size of the affected breast.

4. Patients should be clearly explained the possibility of re-excision if microscopic disease is present on histological examination.

5. Patients should be aware that there is a slightly higher rate of local recurrence following wide local excision and radiotherapy as compared to mastectomy but overall survival is not altered.
